Job Description

Play a crucial role in supporting and assisting individuals living with a disability to navigate the complexities of the NDIS and make the best use of the supports available in in their NDIS Plan.

Attributes required to meet the responsibilities of the position:

* Qualification in disability services, behavioural sciences, social work and/or a human services related field (preferable)

* Industry experience in Support Coordination, case management or other relevant industry experience that demonstrates the ability to engage with and support people living with a disability to access appropriate supports to meet the goals identified in their NDIS Plan

* Strong interpersonal skills and ability to communicate effectively with diverse stakeholders

* Excellent organisational and time management skills

* Conflict management and resolution skills

* Commitment to promoting the rights, independence, and inclusion of people living with a disability

* A strong sense of professional boundaries and integrity

* Queensland Open Driver's Licence

* First Aid and CPR Certificates or ability to acquire Screening

* Working with Children Blue Card

* NDIS Worker Screening Check

* Ability to meet and maintain any other current or future legislative benchmark requirements and government or organisation mandates

* Cultural Awareness- Ability to respond in a culturally sensitive way to all stakeholders and uphold cultural protocols

* Time management and IT competency

* Availability and commitment to participate on an out of hours on call roster if required

* Flexibility to adjust hours when required to meet the needs of participants

The primary responsibility of the Support Coordinator is to work closely with participants, their families, carers and other relevant stakeholders to understand and use their NDIS Plan to achieve their identified goals; connect with relevant support services, including but not limited to allied health professionals and NDIS providers; and build participant confidence to independently use and coordinate their supports.
