About the Role:
A leading community aged care provider is seeking an experienced and compassionate Registered Nurse Case Manager to join their dedicated team in Melbourne. In this vital role, you'll support older Australians by managing a caseload of clients, developing person-centred care plans, and providing holistic support tailored to their medical, emotional, and social needs.

As a trusted advisor to clients and their families, you'll work in collaboration with a multidisciplinary team to ensure high-quality, consistent care. This is a rewarding opportunity to make a meaningful difference in the lives of those you support.

Key Responsibilities:

  • Conduct comprehensive assessments of clients' health and social care needs
  • Develop, implement, and review individualised care plans aligned with best practices and client goals
  • Coordinate services with internal and external providers including GPs, allied health professionals, and community services
  • Advocate for client wellbeing, rights, and access to resources and supports
  • Provide clinical guidance, emotional support, and health education to clients and their families
  • Maintain accurate and up-to-date documentation in accordance with regulatory standards
  • Monitor outcomes and adjust care plans to reflect changing client needs

About You:

  • Current AHPRA registration as a Registered Nurse (essential)
  • Experience in case management, preferably within aged care or community settings
  • Strong interpersonal and communication skills with a compassionate, person-centred approach
  • Excellent time management and organisational skills
  • Understanding of the Aged Care Quality Standards and community care frameworks
  • Ability to work independently and as part of a multidisciplinary team
  • Current driver's licence and access to a reliable vehicle (some travel required)
