We are presently working with a well-respected not for profit organisation who are dedicated to servicing their local communities with the provision of quality, responsive and compassionate care.
We currently have an opening for an experienced Senior Team Leader to oversee the delivery of Home Care Packages based in Sunshine.
Key Responsibilities:
- Manage the day to day operations of the delivery of the HCP program within your port folio
- Guide and supervise the team of Case Managers - ensuring all requirements of HCP program are met at the highest level of service - including assessment, monitoring, reviews, care planning, funding, budgeting, referrals and changing legislative requirements.
- Report into the Senior Manager on all aspects of the delivery of service of the HCP program
Prerequisites required for the role:
- Tertiary qualification in social Work, Allied Health, Nursing or other related field
- Strong Management and Leadership experience essential
- Comprehensive working knowledge and experience with HCP Legislation and Funding
- Solid understanding of Consumer Directed Care models and Commonwealth Aged Care reforms
- Experience in implementing service delivery changes and associated processes
- Proven experience in leading a team to deliver client outcomes
- Excellent time management, communication and negotiation skills
