Physiotherapist/Ex Phys & Occupational Therapist - Empower Lives, Create Independence

Working across home and community settings, you'll play a key role in a multidisciplinary team, helping clients achieve their goals and thrive in their chosen activities.

Your Role:
- Client-Centred Therapy: Deliver individualised therapy services tailored to client needs, using evidence-based and person-centred approaches.
- Assess & Plan: Conduct assessments, develop intervention strategies, and track progress to help clients reach their goals.
- Diverse Service Delivery: Provide Physio/OT support across home, centre, and community settings to promote independence.
- Collaborate & Innovate: Work alongside a team of dedicated healthcare professionals to deliver holistic care.
- Program Development: Contribute to therapy program development and evaluation to meet evolving client needs.
- Smooth Transitions: Assist in discharge planning and refer clients to community programs where appropriate.
- Stay Organised: Maintain accurate and timely documentation in line with professional standards.
SKILLS/EXPERIENCE
- Tertiary qualifications in Allied Health + AHPRA registration
- Experience working in a multidisciplinary team with a strong focus on person-centred care
- Advanced clinical skills in caseload management and treatment planning
- Excellent time management and organisational abilities
- Strong verbal and written communication skills
- National Police Check & Working With Children Check (or equivalent)
- Current driver's licence and willingness to travel across Victoria & Tasmania as needed
- Compliance with workplace vaccination requirements
