About the role
There's an amazing opportunity for a full-time permanent, NDIS Support Coordinator role, to support NDIS participants based in the Ipswich area.
Work directly with people living with disability and their families, and help them maximise the funding that the NDIS has provided.
You will be able to deliver the following:
- Support participants to understand and create their NDIS plan, goals and funding access.
- Plan and coordinate participants to access support services in accordance with their allocated NDIS funds/budgets.
- Connect, establish and maintain participants with support services to pursue/achieve their goals.
- Use NDIS systems and processes to achieve the most of NDIS plans for the participant.
- Montior, support, track and review participants NDIS plans/goals.
- Complete and provide reports to the NDIA with outcomes and success indicators relating to the participants plan/goals.
About you
To be successful in the role, you will possess:
- Experience as a NDIS Support Coordinator (highly desired).
- Hold a current Queensland Blue Card (or willing to apply).
- Hold a current NDIS Workers Screening Clearance (or willing to apply).
- Hold a current QLD Drivers Licence and have your own vehicle (mandatory).
