- Open to 4 or 5 days a week

We are currently recruiting for NDIS Support Coordinators, to support people under the NDIS.
Assist people to strengthen their abilities to connect with relevant services, to develop and maintain supportive relationships and to increase their capacity to navigate through the challenges involved in daily living. Provide flexible support to clients and their families/carers ensuring each client is given the best opportunity to achieve a full and meaningful life.
This role will include:
- Receiving requests, assess and provide appropriate support and advice.
- Accepting referrals from providers, individuals, carers seeking clinical evidence to complete the access process, undertaking a pre-screen to determine likely eligibility for the NDIS and offering support to complete the access process, as required.
- Collecting and collating all necessary evidence and completing the Access Request Form with the consumer and their supporters, in collaboration with the person's (past and present) treating health professional,
- Assisting the person to submit their Access Request Form to the NDIA and acting as a contact person for NDIA queries, at the request of the person,
- Participate Fully as a Team Member
- Working with Community Partners
- Maintain Records and Resources
You will have:
- A valid and current Australian Driver's license
- A valid and current Working With Children Check
- Great report writing skills
- Uphold ethical behaviour, consistent with the values of honesty, fairness, responsibility and hope
- Demonstrate integrity, credibility and foster open communication
- Experience and knowledge of the NDIS
You will also be willing to undergo police and Working with Children Checks, as well as providing 2 recent managerial references.
We will not be considering any applications without sufficient NDIS experience.
