Job Description

A Specialist Support Coordinators' accountabilities may vary in response to the needs of the participant and the goals to which
they have established in their NDIS plan. Regular daily duties of the position may include (but are not limited to):

  • Make genuine attempts to contact the participant within two days after handover with the planner and meeting the participant within five days of the contact depending on the participant's availability.
  • Choose and recommend preferred options or providers and present to the participant the reasons for the recommendation
  • Engaging with service providers as a first point of contact on the participants behalf and coordinating with them to ensure service bookings are established.
  • Arrange any assessments to determine the nature and type of funding required (e.g., complex home modifications).
  • Assess and document choices of mainstream, community, informal and provider options for participants and whereapplicable link the participant (i.e., housing, education, transport, health).
  • Assess the budget for each support type and advise the Plan Manager (Including: 'Self' Manager) of the breakdown of funds.
  • Liaise with the Plan Manager to establish the appropriate claim categories and delegate the correct amount of funds.
  • Assist the participant to get ready for their plan review by helping them review whether they achieved their goals and received value for money for their plan, identify solutions to problems experienced in implementing the plan and consider new goals.
  • Each quarter review budget versus actual expenditure, taking corrective actions where necessary and ensure progress toward achieving client plan goals is adhered to.
  • Strengthen and enhance the participant's capacity to coordinate supports, self-direct, manage supports and participate in the community.
  • Provide your manager for review; reports on specific goals, outcomes, and success indicators within the agreed NDIA reporting frequency.Achieve minimum 85% efficiency of billable hours utilising the Toggl platform.

REQUIRED SKILLS AND ATTRIBUTES
MANDATORY

  • A minimum of two years' experience in one or more from the following: Support Coordination, Support Connection, Local Area Coordination, NDIA Planning, Disability Support Worker, Mental Health Worker, Peer Worker, Welfare Worker, Developmental Educator, Social Worker, Aboriginal Health Worker, Allied Health, Counsellor or Psychologist.Experience working with complex participants.
  • Experience working with people with complex participants.
  • Experience working with people with exceptionally high needs.
  • Experience working with people in crisis management.
  • A demonstrated ability to work with issues that are time critical.
  • Previous experience working with participants involving high-risk and significant barriers to support.
  • Technology literate including Microsoft 365 applications, in particular Word, Excel, PowerPoint, and Teams.
  • Able to demonstrate a sound understanding of the NDIS and disability issues in the Australian context.
  • Strong interpersonal and communication skills, including both written and verbal.
  • Awareness of providers within the local geographic region for NDIS support.
  • Previous report writing experience.
  • Sensitivity surrounding confidential matters is essential.
  • Understand the role of the mainstream service system and the responsibilities within the NDIS sector.
  • Understand the NDIS and operational guidelines and associated legislation.
  • Be fully acquainted with rules relating to participant planning and reasonable and necessary supports.
  • Understand the NDIS Price Guide and flexibility within budgets.
