Mobile Support Worker (Full-Time, Office Hours)
Are you passionate about making a difference in the lives of those experiencing or at risk of homelessness? We're seeking a Mobile Support Worker to join our team, delivering in-home/ on-the-road support for clients throughout their journey towards stability and independence.
About the Role: As a Mobile Support Worker, you'll provide personalised, end-to-end support, meeting clients where they are at- to offer case management, practical assistance, and support tailored to individual needs. Your responsibilities will include conducting needs assessments, safety planning, and helping clients access essential services and stable housing, fostering connections with community resources, and advocating for their needs.
Key Responsibilities:
- Provide in-home, holistic support for clients from intake through exit.
- Develop and implement client-centred case management plans.
- Conduct risk assessments, safety planning, and link clients with necessary services.
- Empower clients towards independence and resilience through proactive, compassionate care.
Qualifications and Experience:
- Minimum Certificate III in Community Services or related field.
- Proven experience in case management, especially with homelessness or housing services.
- Strong understanding of relevant legislation and confidentiality requirements.
