Job Description

Case Manager/ Connection and Development Coach
We're on the hunt for a highly adaptable, heart-led Case Manager who can jump in and support people at risk of or experiencing homelessness.

This is for someone keen on meaningful work and problem-solving that actually matters.

The role will be locum on a month-to-month contract (for at least 3 months) with the potential for extension, and even opportunity for a permanent transtition.

Hours: 4 to 5 days per week (30-38 hours), on Pay Level SCHADS 4 to 5, depending on qualifications and experience.

About the role:
You'll be joining a boots-on-the-ground team, helping clients to:

  • Identify and prioritise their own needs and goals

  • Navigate the web of systems and services

  • Coordinate emergency relief and make referrals

  • Create meaningful, goal-directed support plans

  • Advocate for their rights and transitions to longer-term support

  • Facilitate better wellness, housing access, and human dignity

You'll be mixing practical crisis support with human-first, recovery-oriented planning - all while collaborating across services at the centre.

You're someone who:

  • Has a tertiary qualification in human services, social work, behavioural science (or relevant experience)

  • Understands trauma-informed, culturally safe, strengths-based work

  • Knows how to juggle a caseload, a diary, with interruptions in a fast-paced environment

  • Can build rapport in five minutes and write a brilliant case note in ten

  • Gets that boundaries, good coffee, and self-care are essential

The essentials:

  • At least 2 years' experience in a similar role

  • QLD Driver's Licence

  • Blue Card & National Police Check

  • Experience in housing/homelessness

  • Ready-to-roll attitude and emotional maturity
