Job Description

Tradewind Australia partners with service providers across the Aged Care sector to offer quality care to Victorian communities.

We are seeking experienced Registered Nurses with a background in Care Advisory roles within the Home Care Packages (HCP) program who are open to either Locum or Permanent roles in various locations across Melbourne.

Key Responsibilities:

  • Comprehensive Assessments & Care Planning:
    Conduct clinical assessments and develop tailored care plans based on the clients' medical, personal needs, goals and preferences.
  • Clinical Oversight & Service Coordination:
    Ensure the safe, high-quality delivery of care by overseeing clinical services and coordinating with a multidisciplinary team.
  • Monitoring & Adjusting Care:
    Regularly review and adjust care plans to accommodate any changes in the client's health status or care needs, ensuring optimal outcomes.
  • Collaboration with Healthcare Providers:
    Work alongside GPs, allied health professionals and other service providers to deliver coordinated, person-centred care addressing both clinical and non-clinical needs.
  • Compliance & Documentation:
    Maintain accurate, up-to-date clinical documentation and ensure compliance with Home Care Package guidelines and regulatory standards.
  • Education & Support:
    Provide clinical guidance and support to care staff, clients and families to ensure safe and effective care delivery.

Key Requirements:

  • Bachelor of Nursing and current AHPRA registration
  • Proven experience in a similar role within the Aged Care sector
  • Extensive knowledge of Home Care Packages (HCP), aged care support systems, and funding mechanisms
  • Strong understanding of the challenges faced by older people, their families, and carers
  • Experience delivering person-centred care and supporting models that promote client empowerment
  • Excellent interpersonal skills with the ability to work in a multidisciplinary team
