Position Overview:
Are you passionate about making a difference in the lives of individuals with complex needs? We are seeking a proactive and skilled Forensic Case Manager to join our clients team, working to support individuals requiring tailored disability services. Your role will focus on fostering independence, promoting community reintegration, and delivering high-quality care and support to individuals exhibiting challenging behaviors.

About the Role:
As a Forensic Case Manager, you will draw on your extensive understanding of the NDIS to manage and oversee the delivery of support services for participants, including those with complex care needs. You'll play a pivotal role in ensuring smooth daily operations, coordinating team efforts, and maintaining a client-focused approach.

Your responsibilities include:

  • Building strong relationships with stakeholders such as Justice, Courts, and Community Corrections to facilitate successful community reintegration for participants.
  • Managing participant services by developing tailored support plans and goal-oriented strategies.
  • Providing assertive outreach, intervention, and support for at-risk individuals while maintaining detailed records and case notes.
  • Leading and guiding a team to deliver exceptional care and uphold participant rights to exercise choice and control over their support services.
  • Managing incidents, preparing reports, and working within legislative frameworks.

This role is a blend of direct participant interaction, team leadership, and administrative duties, requiring excellent communication and organisational skills.

About You:
You are a resilient and empathetic professional with a strong background in case management, disability support, AND forensic work. You thrive in dynamic environments, enjoy building positive relationships, and possess the ability to remain calm and assertive in challenging situations.

Key Qualifications and Skills:

  • Diploma or higher qualification in Social Work, Community Services, Psychology, Justice, or related fields.
  • Experience working with individuals exhibiting complex or challenging behaviors.
  • Strong knowledge of NDIS services, legislation, and participant rights.
  • Ability to develop effective support plans and goal-focused strategies.
  • Demonstrated capacity to manage competing priorities and set boundaries effectively.
  • Excellent written and verbal communication skills, with experience in report writing and data management.
  • Understanding of legislative frameworks and risk management in case management.
