Job Description
Leading government body is seeking a highly organized, detail-oriented, and compassionate Case Coordinator with corrections experience to join our team for an immediate start for a five-month contract. The ideal candidate will play a key role in managing and supporting individuals involved in the correctional system, working closely with a multidisciplinary team to provide comprehensive case management and ensure compliance with rehabilitation programs.
Key Responsibilities:
- Have strong experience in case coordination within the Human Services sector. This includes conducting needs assessments and developing and implementing care plans to assist clients in enhancing their independent living skills. Committed to supporting clients in maintaining their eligibility for necessary support programs.
- Capable of managing clients with complex physical and mental health issues and have a clear understanding of their health challenges and treatment plans. You will excel at building strong rapport with a diverse range of clients, including those from corrections backgrounds.
- Previous working experience in completing comprehensive, high-quality assessments that evaluate financial, health, housing, social, vocational, and educational status
This position is a five-month contract that starts immediately. Interviews will take place next week via Teams, and the start date is immediate. Only candidates with prior experience in corrections will be considered due to the complex nature of the role.
