Locum Case Manager - Housing Support & Tenancy Sustainability

ASAP start until end of June 2025

Full time hours, short term casual contract- SCHADS 5

  • Work directly with individuals and families to prevent homelessness
  • Provide wraparound support through strengths-based case management
  • Be part of a mission-driven team creating housing stability and community connection

We're looking for a compassionate and committed Case Manager who's ready to make a tangible difference in the lives of people at risk of homelessness. In this vital role, you'll walk alongside individuals and families to help them sustain their tenancies, navigate challenges, and build resilience.

The program is built on early intervention and holistic support. You'll be part of a values-led, trauma-informed team that champions dignity, independence, and inclusion.

Your Day Might Include:
* Collaborating with clients to create person-centred case plans
* Advocating for services, housing, and wraparound supports
* Linking clients to mental health, DV, AOD, and financial services
* Writing sharp, meaningful case notes and delivering clear reporting
* Building strong referral pathways with local housing and community partners
* Promoting safety, wellbeing, and tenancy stability every step of the way

Who You Are:
You're degree-qualified in human services (or similar), with experience working with individuals and families facing housing instability. You're empathetic and organised, confident in trauma-informed practice, and bring a positive, proactive energy to the people and communities you serve.

You'll Bring:
* A clear understanding of case management frameworks
* A trauma-informed, person-centred approach
* Experience working with people impacted by mental health, DV, financial stress, or AOD
* Strong networking and interpersonal skills
* A current Blue Card, First Aid Certificate, and Driver's Licence
