Support & Advocacy Case Manager - Housing and Homelessness
6 month contract, ASAP start, SCHADS level 5

We are seeking an experienced and motivated Case Manager to provide holistic, trauma-informed, client-centred support through case management, advocacy, and collaboration with stakeholders.
What you'll do:
- Deliver tailored case management plans to meet diverse client needs.
- Work collaboratively with families and individuals to provide practical support, advice, and advocacy.
- Foster strong partnerships with service providers and stakeholders to optimise outcomes.
- Maintain accurate case notes and reporting to meet program objectives.
- Supervise and support volunteers or placement students as needed.
What you'll bring:
- Degree in human services or a related field.
- Experience working with individuals and families experiencing complex challenges like mental health, DFV, financial stress, or barriers to education/employment.
- Strong interpersonal and communication skills, with a commitment to social justice and inclusion.
- Knowledge of trauma-informed, person-centred case management practices.
- Current driver's licence, Blue Card, and current police check.
