We are seeking a passionate Case Manager to join their team and support clients in managing their Home Care Packages (HCP).
About the Role
In this role, you will:
* Develop and manage individualized care plans tailored to clients' needs
* Assist clients in accessing services that promote independence and well-being
* Regularly review and adjust care plans to align with changing circumstances
* Advocate for clients to ensure they receive the best possible care
* Ensure compliance with government regulations and funding guidelines
About You
- Experience in case management, aged care or community services
* Strong understanding of Home Care Packages and aged care legislation
* Excellent communication and problem-solving skills
* A genuine passion for enhancing the lives of older individuals
* Relevant qualifications in social work, aged care, or a related field
* A commitment to delivering values-driven, person-centered care
