Lead with Impact: Coordinated Care Services Team Manager

Your role will involve strategic workforce coordination, resource allocation, and performance management to ensure seamless service delivery. You will:
* Oversee the delivery of person-centered services, ensuring they are tailored to the individual needs of clients while promoting quality of life;
* Ensure adherence to legislation and accreditation standards related the program within the purview of the role;
* Develop and maintain relationships with key stakeholders, including health services, service providers, community organisations and government agencies, to facilitate seamless referrals and service integration.
* Have understanding of the NFP space, with experience adhering to KPIs and managing budgets;
Additionally, this role provide experts guidance to the team to ensure services meet clinical and quality standards.

To be successful in this role, you will have:
- Tertiary qualifications as a Registered Nurse, Occupational Therapist, Physiotherapist, or Social Worker and AHPRA / AASW registration (or equivalent registration authority);
- 5 years' experience leading a team preferably within the Disaility or Aged Care, Community Health or Social Services Sector;
- Current C class Driver's license (QLD).
- In-depth understanding of the aged care assessment processes; and knowledge of the NDIS system.
- Yellow card /NDIS screening clearance, blue card and current police check.
