Tradewind Australia works across the Aged Care & Disability sectors in partnership with providers of services to the Victorian Communities. We recruit for a range of disciplines on behalf of Not for Profit, private and government organisations.
We are presently working with a reputable organisation who provide quality, flexible, client-centred care and support to people living with a disability and Older Australians.
We currently have an opening for a dedicated and detail-oriented individual to join their Community Team as a Care Manager for their new CHSP program.
Key Responsibilities:
- Managing the Commonwealth Home Support Program (CHSP) across the North, East, and West Melbourne regions, as well as Gippsland, Victoria
- Ensuring the delivery of person-centred care support services to clients in line with Aged Care Quality Standards
- Engage with a multidisciplinary team of support staff in the delivery of services
- Ensuring a smooth administration of new client intake
- Managing the assessments and coordination of services
Prerequisites required for the role:
- Tertiary qualifications in Aged Care, Social Work, Nursing, or a related field (minimum Diploma level; Bachelor's preferred).
- Experience within an aged care environment
- Strong understanding of the Commonwealth Home Support Program (CHSP) and related legislation, as well as the broader aged care regulatory framework.
- Strong Management and Leadership experience essential
- Experience with service navigation, referral management, MAC Portal and data entry
- Excellent time management, communication and negotiation skills
- Current NDIS Working Screening Check and Victorian Drivers Licence
