Job Description
We are seeking experienced ACAS Assessors for a highly flexible and autonomous role. Based in Hastings, however this position offers the flexibility to work remotely, with requirement for home visits across the Casey and Cardinia areas.
Key Responsibilities
- Conduct phone or face-to-face assessments for clients referred through the My Aged Care Portal.
- Work with each client to establish individual goals, create tailored strategies, and refer to aged care and other relevant services.
- Develop personalised Support Plans to help clients access the support they need.
Role Requirements:
- 6 months of experience
- Relevant qualification in healthcare including Social Work, Occupational Therapy and Nursing Qualifications (For nursing degree, must be registered with APHRA. Cannot accept Enrolled Nurses).
- This role is suited to those who hold current ACAS certification in SOA4 and IAT due to limited training provided.
Skills and Experience
To be considered for this role, you will need:
- Recent ACAS assessment experience, with knowledge of the My Aged Care portal, IAT assessment tool, and HCP/CHSP funding.
- A tertiary qualification in Allied Health, Nursing, or Social Work.
- COVID-19 vaccinations and the 2024 flu vaccination.
